Immunization Requirements

Students entering school must be in compliance with the Minnesota State Immunization Law.

No shots. No School.
It's the law in Minnesota


St. Louis Park Public School students must have immunizations up to date and the school must have the child's shot records for him or her to start school.

  5 5th shot not needed if 4th shot was given after age four years

** An alternate two-dose schedule of Hepatitis B may also be used for kids from age 11 through 15 years.

Has the child had the number of shots shown by the checkmark(s) under each vaccine? Each row is meant to be read separately, so don't add up the columns of checkmarks under each vaccine.
