ImmunizationsMinnesota Statutes Section 121A.15 requires children enrolled in a Minnesota school to be immunized against certain diseases, or file a legal medical or conscientious exemption. This form is designed to provide the school with information required by the law.
A copy of the student's current immunization record must be on file, at school.
Check now with your family doctor or clinic to see if your child(ren) has all the immunizations required for their age. If the student receives an immunization during the summer months send by mail, fax, or drop off at the school the date (day, month and year) of the shot received. If you have questions or do not have insurance contact the nurse at your child's school for assistance.
St. Louis Park Public Schools has joined the growing number of school districts throughout the state of Minnesota who strive for immunization compliance. This allows schools and local clinics working together to see that all students in our district are properly immunized before starting school in the fall. This helps provide barrier free access to immunizations for families without insurance and /or a family clinic. More information can be found at here. Parents may get a legal exemption for medical reasons or conscientiously held beliefs.The links below provide detailed information on required immunizations and a form for you to complete and return to your health office.
Early Childhood Screening
This is an evaluation to check the health and development of your child, so concerns may be addressed early. This enables your child to be ready to be a better learner in kindergarten. It is a free service for children ages 3½ to 5 years old and is required for entrance into kindergarten. Screening is held September to early May at Central Community Center. If your child has not been screened, call 952-928-6726 to schedule an appointment.
Non-residents of the St. Louis Park Public Schools can apply for Open Enrollment through the state of Minnesota's Enrollment Options Program. For further information about open enrollment or to obtain an application please email email@example.com or call 952-928-6023.Equity Options Busing Program
Until 2012, the Minnesota Department of Education sponsored The Choice is Yours (TCIY) program - a school choice program for families that qualified for free or reduced-priced lunches and lived within the Minneapolis Public Schools attendance boundaries. This open enrollment program offered low-income families from Minneapolis more options to attend schools in outlying suburbs. From the beginning, St Louis Park Public Schools voluntarily participated in this program.
When the TCIY program ended in 2012, St Louis Park Public Schools chose to continue its Minneapolis busing program to provide eligible students access to the district’s quality education options. In 2018, the program was renamed “Equity Options Busing Program”.
To participate in the Equity Options Busing Program, students must meet two criteria:
- The student and parent/guardian must live within the Minneapolis Public Schools attendance boundaries.
- The student must qualify for free or reduced-price lunch.
If both criteria are met, the student is eligible for the Equity Options Busing Program. This open enrollment busing program provides transportation to and from school. If you are unsure of your child's eligibility for free or reduced-price lunch, please visit School Nutrition on our web site at www.slpschools.org.
Registering for Elementary School
Students who move into the St. Louis Park school district during the school year should register as soon as possible at the Enrollment Center, located at 6311 Wayzata Blvd. St. Louis Park has three K-5 elementary schools:
The fourth elementary school serving students in Grades K-5 is Park Spanish Immersion School (PSI), located at 9400 Cedar Lake Road Walker. Families of prospective kindergarten students wanting their child(ren) to attend Park Spanish Immersion School must apply by the deadline to be included in the lottery. The application deadline is identified each year and is published in the city/school calendar. Applications are sent out to families in January and the lottery is held in early February of the year the child will start kindergarten. If you did not receive an application, please contact the Enrollment Center (phone: 952-928-6023). Registering for the Middle School (Grades 6-8)
At the secondary level, the St. Louis Park Middle School serves students in Grades 6-8 at 2025 Texas Avenue South. Please call Bridgid O'Hara at 952-928-6301 to schedule an appointment to register. Please bring a current copy of the child’s:
Registering for the High School (Grades 9-12)

- Proof of residency (this may be a utility bill or a purchase or rental agreement)
- Please bring any most recent transcripts and/or report cards you may have
St. Louis Park serves Grades 9-12 at the High School located at 6425 West 33rd Street. Contact the Student Services Office (phone: 952-928-6123) at the High School to make an appointment to register your student. Please bring a current copy of the child’s:

- Birth certificate*
- Proof of St. Louis Park residency (this may be a utility bill or a home purchase or rental agreement)
- Secondary students are also required to bring a copy of their current transcript

During the summer, parents of children who will be enrolled in Grades K-5, can enroll their child(ren) at the District Office, 6311 Wayzata Blvd., (phone: 952-928-6000). At the secondary level, students at the Middle School (Grades 6-8) should call 952-928-6395 to set up a registration appointment. At the High School (Grades 9-12), contact the Student Services Office at 952-928-6123.
