
Transition Plus
Transition Plus is a comprehensive post-secondary transition program designed for young adults with developmental disabilities ages 18–22. Our program bridges the gap between high school and adulthood by equipping participants with the practical life skills, vocational readiness, and social-emotional tools needed to become confident, independent, and contributing members of their communities.
Our goal is to equip young adults with independent living skills, community engagement opportunities, employability, and self-advocacy so they transition into adulthood as capable, contributing citizens.

Student Success Advisor

This graphic explains our Student Success Advisor program. On the left, it shows a support team made up of a primary advisor, a career coach, a mental health consultant and a family engagement liaison.
On the right, it describes how students are supported over time. Each week, students meet for 30-45 minutes to discuss academics, internships, life skills and personal goals. Every quarter, they take part in a review conference that includes a student presentation, progress data, goal updates and graduation planning.
