Back-to-School Checklist

Update Student Information

If your information has changed, please complete the forms outlined in this section. (If nothing has changed, no action is needed.) Contact the district's Enrollment Center via email or at 952-928-6023 with any questions.

Address/Contact Change

If your changed your address, phone number or email address, please email the Enrollment Center with the following information:

Student's first and last name
School name
Proof of address (e.g., utility bill, lease)

If applicable, also complete the address change open enrollment application form (PDF).

If you have moved outside of the district but would like to stay enrolled, you will need to complete the address/contact change form and the open enrollment application.

Name/Gender Change Request

Please complete the name/gender change request form to let us know if your student has a different name/gender than what is currently on file.

View & Confirm Transportation Information

Parents and caregivers will receive their student's bus route information in late August, once our team has assigned routes to students.

Please update your address to ensure reliable bus routing information, so we can have the correct location for pick up and drop off. Not updating your address information with the school could result in significant delays in getting your student to school.

To update your information, please email the Enrollment Center with the following information:

Immunizations

Minnesota Statutes Section 121A.15 requires children enrolled in a Minnesota school to be immunized against certain diseases, or file a legal medical or conscientious exemption. In addition, a copy of the student's current immunization record must be on file at school. Students will not be able to attend school in the fall if they are not up-to-date with vaccines and do not have a legal exemption on file.

No Shots, No School Program at Creekside Clinic

Through No Shots, No School, HealthPartners collaborates with many local school districts to make it easier for children to have an immunization-only appointment, whether you have health insurance or not, at no direct charge to you.

The program begins every August and is available through the end of October. Students continue to be eligible throughout the school year if they enroll in school or need additional doses of their immunization in order to complete the series. Follow the instructions on the HealthPartners website to make an appointment and complete the necessary forms.

Activities, Athletics & Programs

Community Education & Early Learning

Registration for fall activities (grades K-8) begins in September, and preschool registration begins as early as January. Visit the district's Arux website to register your student for before- and after-school activities and early learning services.

Application for Educational Benefits

All St. Louis Park Public Schools families are encouraged to fill out an Application for Educational Benefits each year. In addition to helping our schools quality for education funds and discounts, families who complete the Application for Educational Benefits can also qualify for benefits such as:

Advanced Placement (AP) tests for potential college course credit
College application fees
Internet access
Sports and activities fees
