Student Engagement Surveys

Instructions for administration:
- Find your student engagement survey. Open the survey and “Make a copy” by clicking the three dots in the upper right-hand corner of the google form, OR right-click and choose “make a copy”. This will create a copy in your own drive. Go to the copy and right-click and choose “get link”. This is the link you will share with your students.

Please check your google form “settings” to make sure that the “collect email addresses” option is turned off. This will protect the anonymity of your students.

Share the link to your survey with your students to begin data collection

Paper surveys are available for students that need IEP or 504 accommodations. Please contact Dr. Becca Starr if you need additional support in this area.

If you need clarification about which survey to administer, please contact our district assessment specialist, Dr. Becca Starr, or your supervisor.

You only need to administer one survey to one class for the year.

Prior to administering the survey, make sure you have the following: Devices for students.

We’ve transitioned the Student Engagement Surveys from Survey Monkey to Google Forms to accomplish two things:
1) to provide you with immediate access to survey results, and
2) to allow you and/or your CARE teams to add additional questions that would be meaningful to your own professional growth and development.

Upload your survey report and your reflection into Truenorthlogic.

The survey is meant to provide you with a snapshot of student opinions and data over multiple years and will help you identify strengths and areas of growth.
